Pet Owner Booking Form

To arrange collection of your pet from either home or vet clinic; or if you would like to drop your pet to our crematorium simply click the green “Click here to make a booking” button below. 

Once the booking form has been submitted we will be in touch as soon as possible to confirm the booking.

If you would like to pay for your pets cremation online, payments are accepted via Credit Card, Afterpay, Osko or bank transfer. 

Please head to our Pay online page to make an online payment after you have submitted your booking request.

Weekends

Our office is not open on the weekend however we are available for emergencies:
Saturday and Sunday 10am to 3pm **

We will try our best to accommodate but there may be times when we are not available at a time that suits you. 

Terms
Local Vet Clinic Collection
Available Monday to Friday 9am to 5pm

You DO NOT need to wait at the clinic for us.

The vet clinic has the facilities to safely store and keep your pet safe until we arrive to collect. 
Please let the vet staff know you have made arrangements with us and they will be able to keep your pet safely stored until we arrive. Their facilities do allow for safe storage overnight or over the weekend if needed.

Collection will be a minimum of 1 hour after your vet appointment time and within 1 business day. If your appointment is booked with your vet at 3pm or later, we will collect your pet on the following business day. 

Vet Appointment for Home Euthanasia – Home Collection

Collection must be a minimum of 1 (one) hour after your scheduled appointment with the vet. We require the minimum timeframe before our arrival as the euthanasia procedure can take up to 45 minutes to 1 hour to perform.

We kindly ask that if the vet is running late and has not arrived 30 minutes before our scheduled arrival time, please give us a call so we can delay our arrival.  

Home Collection

A small fee is applicable for all Home Collections. 
Home collection is available 
Monday to Friday 9am to 5pm
Monday to Friday 5pm to 7pm **
Saturday and Sunday 10am to 3pm **

We will confirm your booking time by return email after the form has been submitted.

CANCELLATION POLICY
We understand that sometimes things can change, the euthanasia does not go ahead, vet appointments are cancelled/rescheduled or you decide to make alternative arrangements for your pet. Therefore, please note if you do not contact us to cancel or reschedule your appointment at least 30 minutes BEFORE our allocated appointment time you will be charged a cancellation fee.
